There is no single duration for wearing compression stockings after thermal ablation of veins. The duration depends on the type and volume of the intervention, the condition of the veins, the severity of postoperative symptoms, and the individual risks of the patient.
What is thermal ablation of veins
Thermal ablation is a minimally invasive method for treating varicose veins, in which thermal energy is applied to the affected vein from the inside. Thermal methods include:
- endovenous laser obliteration — EVLO or EVLK;
- radiofrequency obliteration — RCHO or RCHA.
During the procedure, the doctor inserts a light guide or radiofrequency catheter into the vein. Under the influence of thermal energy, the treated section of the vein closes and is gradually excluded from the pathological blood flow. Thermal ablation can be performed as a standalone intervention or combined with mini-phlebectomy, sclerotherapy, and other procedures. This is one of the explanations for why recommendations for compression after treatment may vary.
Why are compression stockings prescribed after thermal ablation
Medical compression knitwear creates controlled external pressure on the limb. After the intervention, compression may be used to reduce pain, swelling, and discomfort, as well as as part of postoperative rehabilitation. In the Russian clinical guidelines "Varicose Veins of the Lower Extremities", approved by the Scientific and Practical Council of the Ministry of Health of Russia in 2024, the goals of compression therapy include pain reduction and shortening rehabilitation times after invasive treatment. At the same time, compression knitwear does not replace medical supervision and does not guarantee the absence of complications. It is a separate component of treatment, the necessity of which is assessed based on the procedure performed and the patient's condition.
Why it is impossible to specify a single duration for wearing stockings
In clinic recommendations, you can find different durations: from a short period after the procedure to several weeks. However, such schemes cannot be automatically applied to every patient. Clinical guidelines formulate the criterion as follows: after invasive intervention, compression therapy is prescribed as needed, determined by the nature of the intervention. Thus, the document does not establish the same number of days for all patients.
What does the doctor consider
The duration and regime for wearing compression knitwear may be influenced by:
- the type of thermal ablation — laser or radiofrequency;
- the number of treated veins;
- the duration of the intervention;
- the simultaneous performance of mini-phlebectomy or sclerotherapy;
- the presence of postoperative swelling, pain, indurations, and hematomas;
- the initial condition of the veins and the clinical class of the disease;
- the presence of trophic skin changes;
- the state of arterial circulation;
- previous thromboses and other individual risks;
- the results of follow-up examination and ultrasound study;
- the fit of the garment and tolerance of compression.
This list explains why two patients after seemingly similar procedures may receive different prescriptions.
Duration of wearing and wearing regimen — different questions
Even if the doctor specifies the overall duration of using the garment, it is necessary to clarify the regimen separately:
- whether the stockings need to be worn continuously in the first hours or days;
- whether it is allowed to remove them at night;
- whether the stockings need to be put on before getting out of bed;
- whether compression is required on one or both legs;
- whether the regimen changes after the follow-up examination.
The answers depend on the specific clinical situation. It is not advisable to independently shorten or extend the prescribed period, switch to another class of compression, or replace the garment with an elastic bandage.
What stockings can be used after thermal ablation
After the intervention, the doctor may prescribe medical compression knitwear designed for an active patient. It should not be automatically replaced with hospital (anti-embolic) stockings. Hospital knitwear is primarily intended for surgeries, inpatient care, and periods of limited mobility. Everyday medical compression knitwear is designed for use in walking conditions, working the muscle-venous pump of the calf, and regular daily activities.
Compression stockings do not replace postoperative monitoring
After thermal ablation, the doctor may prescribe a follow-up examination, ultrasound study, and other measures. Compression stockings do not replace these stages. It is important to distinguish between compression therapy and the prevention of venous thromboembolic complications. The decision on pharmacological prevention is made by the doctor based on individual risk. The presence of stockings is not a basis for independently canceling prescribed medications or other measures. If after the intervention there is increasing pain, pronounced swelling, color change of the limb, shortness of breath, chest pain, or any other sharp deterioration in well-being, timely medical assistance is required.
